SPORTS PHYSICALS - Wednesday May 20 - 12:30 pm
This year OHSAA is asking you to fill in the information online through the link below.
Dr. Rick Thompkins will be here Wednesday, May 20 beginning at 12:30 to do the physical examination. All physical information should be done online or a completed physical packet will need to be brought at the time of the physical. Payment of $10 cash will be required at the time of the physical.
If you have any problems or questions, please call the high school office at 330-927-7141.
All students are required to have a physical on file in order to participate in athletics. This year you can fill the information for the physical through the PRIVIT website. The link is
